• Successful Primary Care group hiring - North Palm Beach, Florida
  
• Location: Jupiter (10 min from the beach)
  
• Outpatient Quality Care / Managed Care
  
• In Office - True Outpatient only position
  
• Monday through Friday 8:00am - 5:00pm
  
• No weekends
  
• Low patient panel

• Comfortable with use of Electronic Medical Records (EcW)
  
• Detect and manage chronic diseases
  
• Develop patient strategy to reduce hospitalization and ER visits
  
• Collaborate with Case Managers and Specialty Providers
  
• Educate patients on the value of compliance
