Education Specialist (CDCES) Monte Nido 1099/Contract Role Remote
Monte Nido has been delivering treatment for eating disorders for
over two decades. Our programs offer a model of treatment that
blends medically sophisticated care with a personalized treatment
approach. Our work is grounded in evidence-based strategies for
adults and adolescents suffering from eating disorders. We work
from a multi-disciplinary treatment team approach while integrating
state-of-the-art medical, psychiatric, nutritional, and clinical
strategies to provide comprehensive care within an intimate home
setting. We are seeking a Certified Diabetes Care and Education
Specialist (CDCES) join our team on a remote, contract basis
working 8-15 hours/week census depedent. Under direction of the VP
of Adolescent Medicine the Certified Diabetes Care and Education
Specialist (CDCES) acts as an interface between client, home team,
and medical team to guide optimal diabetes care while in treatment
at Monte Nido. CDCES will guide insulin and other medication
management in direct coordination with medical team, as well as the
rest of the care team in reviewing glucometer readings, optimizing
diabetes management, and educating patients on any lifestyle
changes recommended. The employee is responsible for establishing
and maintaining interpersonal relationships with patients, family,
home team, and staff in a courteous, respectful and professional

status. Responsibilities Include:: 1. Serves as Quality
Coordinator, providing oversight of Diabetes Self-Management
Education and Support MNA services, ensuring: - Implementation of
the standards - Services are evidence-based. - Service design
incorporates population needs. - Ongoing service evaluation and
continuous quality improvement plan is reviewed at least annually.
- Collection and evaluation of data to identify gaps in services -
Keeping up to date with most current interventions and devices 2.
Educates patients on necessary lifestyle changes involved in
diabetes diagnosis and management, including nutrition, blood
glucose levels, medications/insulin, or pump management when
prescribed by medical staff. 3. If the sole role is diabetes
management, collaborate with other assigned disciplines. Work with
RD in the management of meal plans and physical activity. Work with
the medical team in the management of blood sugars and medication
changes. 4. Performs data collection if needed to use for
assessment and patient management (i.e., blood sugar monitoring
/monitoring of CGMs) 5. Facilitate diabetes and nutrition education
classes. 6. Provide family support as needed. 7. Provide in-service
training to MNA departments as needed. 8. Maintains a library of
accurate and appropriate education materials as an adjunct to
counseling and education. 9. Demonstrates ability to document in
the patient record in an accurate, concise, clear, legible, and
organized manner that reflects assessment, interventions, and
plans. 10. Ability to dine with patients as scheduled. 11.
Participate in staff meetings when asked; responsible for obtaining
information if not in attendance. 12. Participates in committees as
directed by supervisor. 13. Seeking out learning experiences and
incorporates new knowledge into practice by attending and/or
presenting at local and national conferences as approved and when
opportunities arise. 14. Responds and assists in emergency
situations as needed in a manner appropriate to training and
experience. 15. Approaches patients/families/professionals and
visitors with a respectful, caring manner to convey a sense of
professionalism, concern, and warmth. 16. Maintains confidentiality
of patient information and records. 17. Communicates and
problem-solves collaboratively with all team members. 18. Delivers
care in a non-judgmental and non-discriminatory manner, sensitive
to patient diversity. 19. Seeks constructive criticism and has the
ability to evaluate suggestions objectively. 20. Attends
in-services and educational training as necessary. 21. Maintains
flexibility and adaptability to expected or unexpected changes in
the work environment. 22. Reports incident occurrences in
accordance with policy and procedure. 23. In an emergency or
declared state of emergency, may be required to stay connected
virtually to help with urgent concerns. 24. Performs all other
tasks as assigned Qualifications:: - Graduation from an accredited
college or university with a minimum of a Bachelor’s Degree in
Nutrition or a related field of human services. - Three years of
Clinical experience preferred - Experience facilitating behavior
change in the area of eating disorders/mental health preferred -
Certified Diabetes Care and Education Specialist Credential -
Registered Dietitian or a Registered Nurse - CPR certification may
be required based on state regulations. - Knowledge of the
Principles and Practices of the discipline. - Knowledge of
nutritional sciences and current treatments for diabetes and
related manifestations - Experience giving presentations and
facilitating group classes - Skilled use of Electronic Medical
Records - Ability to work both independently and in a team
environment
